Case Management and Mental Health Services

Integral Community Services partners with Integral Healthcare and Therapy Services to provide coordinated case management and mental health services that support individuals and families in achieving stability, wellness, and long-term success. Through this partnership, clients receive comprehensive assessments to identify health, social, educational, and economic needs, followed by individualized care planning based on their goals and priorities.

Our case management services include care coordination and navigation across healthcare, social service, education, and community systems; connection to essential resources such as healthcare, housing, food assistance, employment, education, and transportation; and referrals to appropriate programs and agencies. We also provide advocacy and hands-on support with applications, documentation, and communication with service providers to help reduce barriers to care.

In addition, clients receive education and skill-building support to better understand available services, benefits, and rights, empowering them to make informed decisions and strengthen self-advocacy. Ongoing monitoring and follow-up ensure services remain effective and responsive as needs change, with plan updates as appropriate. When urgent situations arise, we assist with crisis support and linkage to emergency and community resources to ensure timely, coordinated care.
